I kept it simple because, you know, it's for a dude. There's a ton of free cd templates out there, but if you want mine you can download it here. I printed it out on a sheet of sticker paper and then stuck it to heavy weight decorative paper and cut it out. Fold the flaps down first, slap some glue on them and then fold the whole thing in half.
